Medical Insurance Physicians and Nurses: ECPS offers health insurance through Blue Cross Blue Shield, per the below. You must choose between a PPO plan and an HMO plan.  PPO     Single person   : Based on Application Employee and child : Based on Application Employee and spouse   : Based on Application Family : Based on Application       HMO     Single person   : Based on Application Employee and child : Based on Application Employee and spouse   : Based on Application Family : Based on Application Dental cost is based on your individual application. If enough employees want a dental plan, then we will add it to the medical plan. The physician visit co-pay cost is based on your application for in-plan office visits. For all medical services not provided in the physician's office, you would pay according to the insurance plan of choice. The above deductibles (based on application) include all in- and out-patient surgeries and non-covered ancillary services.  There is a prescription card that is based on the plan you choose.
